Given the return of universities to in-person instruction starting December 1st and the requirement to present a COVID vaccine card for attendance at the university and in dormitories, with the support of the Information Technology unit at Qom University of Technology, you can log into the system (LMS) and complete the related form and upload the necessary documents by November 25th, to assist the university officials in making precise and appropriate arrangements.
It is important to note that dear students can log into the system using their national ID, old foreign students with their foreign ID, and new foreign students with their passport number.
It should be noted that the size of the vaccine card photo or health issue document to be uploaded in the vaccine form must not exceed 1 MB. The image file formats can be png/jpg/jpeg. PDF screenshots of the related documents can also be uploaded using a mobile phone or laptop.
