Supplementary Insurance Plan for Students

Required Documents for Registration of Supplemental Insurance  (Bimeh Day) Second Semester 92-91:

1.  Copy of the first page of the birth certificate
2. Copy of the national ID card
3. One recent photograph with a note on the back
4.  Copy of the first and second pages of the healthcare insurance booklet or social security insurance
5. The last date for registration and payment of fees is 91/12/9
6.  The annual supplemental insurance premium (from Bahman 91 to Bahman 92) is 760,000 rials
7. Account number for depositing funds: 44505790/53 Bank Mellat    Account holder: Bimeh Day  
8.  Students can utilize insurance loans for payment of the supplemental insurance premium.

Note: It is essential for the student's name to be included as the payer on the deposit slips.
