Attention to colleagues who wish to use the supplementary insurance services:
* All individuals who had supplementary insurance during the previous period of 97-98 and wish to join the new period must complete the registration form. (Membership from last year does not automatically renew it, and interested employees should take the necessary actions for the new period.)
* The contracted insurance company for supplementary insurance is the Teacher's Insurance Company.
Registration Method
* Interested colleagues can obtain the registration form and, after completing the Excel file, submit it to Mr. Ahmad Ahmadabadi via the ERP system by the deadline of 98/08/27. Given the arrangements made, access for all faculty members and esteemed staff to Mr. Ahmadabadi in the ERP system has been provided.
The primary insured individuals are the full-time employees of the university, including official, contract, and corporate employees.
The contracted insurance from 1398/08/01 to 1399/07/30 by the Teacher's Insurance Company comes with a monthly premium of 570,000 rials per person, along with the attached commitments.
The premium for individuals aged 61 to 70 years is 855,000 rials, and for those over 70 years, it is 1,140,000 rials, and for non-dependent individuals, it is double the premium.
Fifty percent of the premium for the primary insured individuals and the dependents of faculty members and official, contract, and temporary employees will be covered according to previous years, provided funding is available through welfare services by the university.
The waiting period for maternity coverage is 9 months, and for chronic disease group coverage, it is 3 months. It is obvious that individuals who were insured with other commercial insurance companies last year, if they present documented and verified evidence by the previous insurer, will not have a waiting period.
A copy of the contract is attached to the letter.
The account Shah ID number preferably should be from Bank Melli or Bank Mellat.
