Health Monitoring Plan for Staff and Faculty Members
General Objective:
Assessing and improving the health of university staff and faculty members.
Specific Objectives:
- Determining health indicators
- Early detection of diseases
- Timely therapeutic interventions
- Raising awareness for disease prevention
Features:
- Compliant with the guidelines of the Canadian Task Force on the Periodic Health Examination (CTFPHE) and the U.S. Preventive Services Task Force (USPSTF)
- Based on the principles of the Family Physician Plan (referral system)
- The first health monitoring plan implemented at the university (tailored to the needs of the target population)
- Establishment of the first electronic health record with a unique code assigned to each individual
- All phases of the plan are provided free of charge
Implementation Steps:
Sending Invitations
1. Sending invitation letters from the Health Monitoring Unit to all staff and faculty members
Scheduling the date and time for participation in the plan
Reception and File Creation
2.Creating a health file for each participant
Handing over the medical file along with a plan guide sheet
Providing guidance on completing personal information and medical history forms
Issuing a referral letter specifically for the plan to conduct blood tests
Nursing Services
3. Electrocardiography (ECG)
Measurement of height, weight, and blood pressure
Vision Testing
4. Measuring the visual acuity of both eyes
Assessing color vision
Referral to an ophthalmologist if necessary
Audiometry (Hearing Test)
5. for individuals over 40 years of age
Medical Examinations
6.General physical examination of all body systems
Examination for cardiovascular diseases and, if necessary, referral to a cardiologist for consultation and treatment
Referral for supplementary examinations such as ultrasound, mammography, radiology, gynecology, etc., if deemed necessary by the attending physician
Referral to specialized clinics if needed (on a case-by-case basis)
Blood Tests and, if required,
7.other supplementary laboratory tests
Specialist Consultation
Review of the file by a cardiologist and, if necessary, referral for further supplementary tests (exercise stress test, echocardiography, repeat ECG)
Review of the file by an occupational medicine specialist and therapeutic measures if required
Review of the file by other specialists as recommended by the primary care physician
Health Monitoring Unit
9.In addition to providing the above medical services, this unit is also responsible for the following:
- Recording and archiving data
- Reviewing files by a medical records expert
- Preparing and issuing a Health Profile (Health Passport)
- Monitoring the health of staff and assisting in disease treatment
- Educational needs assessment
- Statistical analysis and data interpretation
- Reporting the results and findings of the plan to the authorities for strategic planning purposes
- Investigating and identifying occupational complications and diseases, and striving to improve workplace environmental conditions
- Organizing educational workshops to enhance health knowledge
- Preparing and publishing educational materials, including periodicals, books, and multimedia resources
